What is it?

The Chatham County Hospital Authority (CCHA) Trust announces a $1.2M grant funding opportunity and invites nonprofit and for-profit organizations providing preventative health care services to the indigent population in Chatham County, Georgia, to apply for funding through a competitive grantmaking process. Individual grant awards will not exceed $250,000.

United Way of the Coastal Empire (UWCE), as a seasoned community grantor, will administer the CCHA Trust Grant. While United Way will facilitate the grant process, CCHA will retain full decision-making authority.  

The purpose of the CCHA Trust Grant is to provide preventative health care services to the indigent population of Chatham County, Georgia.  

Interested organizations/entities should review eligibility criteria and instructions below and submit a Letter of Intent (LOI) by 5:00 PM on July 26, 2024. LOI decision letters will be sent August 6, 2024, and agencies/entities invited to submit a full application will have access on August 12 at noon EST through September 4 at 5:00 PM EST. Applicants will be notified of awards in November 2024. The award will be disbursed from January 2025 – December 2025. 


Eligibility Requirements

  • Must address at least one of CCHA’s focus areas:
    • Behavioral and mental health services
    • Health screenings
    • Dental hygiene care
    • Specialty care services (e.g. diabetes)
    • Patient advocacy and navigational resources
  • Must serve indigent Chatham County, Georgia residents
    • “Indigent” is defined, for this purpose, as households at or below 138% of the Federal Poverty Line in alignment with the U.S. Department of Health and Human Services. The current income level for this funding cycle is equal to $20,783 per person or $43,056 for a family of four.
    • Must provide documentation to certify client household income and location.
  • Organization/entity must be a 501(c)(3) nonprofit or for-profit business with accompanying business licensing and certifications.
    • Must provide proof of insurance (e.g. nonprofit liability insurance or for-profit malpractice insurance certificate).
  • Organization/entity complies with the USA Patriot Act and all federal and state laws concerning discrimination based on age, race, religion, gender or gender identity, sexual orientation, economic status, disability, cultural heritage or national origin.
  • For-profit entities must provide:
    • Most recent tax returns
    • Last completed income statement (profit and loss)
    • Balance sheet
    • Last completed audit, compilation, or review 
  • Non-profit entities must provide:
    • Most recent tax returns
    • Last completed income statement (profit and loss)
    • Balance sheet
    • Last completed audit, compilation, or review
  • Organization/entity complies with all CCHA Trust eligibility requirements and bylaws.
  • Organization/entity has a conflict-of-interest policy.
  • Grant funds cannot be used for capital campaigns or fundraising events.
  • As a condition of accepting funding, Grantees must report data and outcomes through a mid-year and final report.

How to Apply

Eligible organizations/entities interested in applying for CCHA Trust grant funding must first submit a Letter of Intent (LOI). Submitting an LOI does not guarantee an invitation to submit a full grant application. 

The LOI should reflect anticipated service delivery within the CCHA Trust grant period of January 1, 2025 – December 31, 2025.

About the Chatham County Hospital Trust

The Chatham County Hospital Authority Trust (“Trust”) was established as of November 28, 2017, to provide health care for the indigent residents of Chatham County, Georgia. The Trust will be operated to comply with the relevant provisions of Official Code of Georgia Annotated (“O.C.G.A.”) Section 31-7-70, et seq., including but not limited to the requirements of O.C.G.A. Section 31-7-75.1 (“Hospital Trust Statute”)The Trust is exempt from federal income tax under the provisions of Section 115 of the Internal Revenue Code of 1986, as amended.

Chatham County Hospital Authority was created as a public body corporate pursuant to provisions of the Hospital Authorities Law of 1941 and established and reconstituted by Chatham County under resolutions of Commissioners of Chatham County, dated October 17, 1952, October 24, 1952, and November 6, 1954.
